Tattooing of Minors

Florida State and Santa Rosa County Law allows for minors, ages 16 and 17 only, to get tattoos with parental consent. If you are the parent of a minor wanting a tattoo, there are a few steps you will need to take.

Start by clicking below and it will take you to the health department website form. It will need to be printed out, or you can stop by the shop and we have copies at the front desk.

THE CONSENT FORM MUST BE NOTARIZED IN SANTA ROSA COUNTY ONLY!

We will not be able to use consent forms from notaries in any other county.

 Don’t sign forms ahead of time, as the notary must be present. We are unable to do any notarizing at the shop for you unfortunately. None of us is a notary for starters, but it is also a conflict of interests. We recommend the UPS store in Navarre about a mile away from our shop, the cost is $10. You will need to present photo ID for guardian AND for minor. If your names and addresses don’t match on the ID, you will need to present a birth certificate. Bring all of this to tattoo appt. Our studio also has waiver forms you will need to fill out so please arrive 10 min early with photo ID.
